Strengthening home health services to better support patients beyond the hospital

Later this calendar year, UI Health Care will bring its affiliate, UI Community HomeCare, together with its home health services to create a single, unified home health program. This integrated model will bring skilled home health nursing, home infusion, durable medical equipment, and medical supplies under one coordinated program, helping patients transition safely, smoothly, and confidently from hospital to home. 

“This is part of a strategic effort to broaden the scope of services that serve our community while delivering exceptional home care to our patients,” says Brad Haws, MBA, chief executive officer of the clinical enterprise and associate vice president of University of Iowa Health Care. “When patients can be discharged with confidence, knowing the right services will be in place at home, it improves the patient experience, supports timely discharges, and helps reduce hospital readmissions.”  

UI Health Care’s home health program was recently recognized by U.S. News & World Report as one of the nation’s highest‑performing home health agencies, a distinction that reflects the expertise, dedication, and compassion of our home health professionals. 

By bringing the two separate home health services together, UI Health Care aims to strengthen collaboration across care settings and improve coordination from inpatient care to recovery at home and in the community. 

All UI Community HomeCare team members in good standing will be offered positions within UI Health Care, and there are no immediate changes to operations or clinical services at this time.
